Job Title: Case Manager

Job Summary:

Lao Family Community Development, Inc. is seeking a skilled Case Manager to join our team. As a Case Manager, you will provide field-based case management services to newly arrived and resettled refugees. Your primary responsibility will be to build strong relationships with refugee clients, helping them stay engaged in their case plan and removing barriers to needed services.

Key Responsibilities:

  • Provide culturally and linguistically appropriate case management services to refugee clients.
  • Conduct education and outreach initiatives to inform refugees about resettlement and immigration services, community resources, and local systems.
  • Perform individualized case management assessments to address the unique needs of each client.
  • Offer comprehensive family support services, including enrollment in school for minor children, obtaining identification cards, navigating local services, and accessing public benefits.
  • Develop personalized financial plans with clients to empower them to manage their household finances.
  • Facilitate job readiness training and employment placement services, collaborating with local employers to identify job opportunities suitable for clients' skills and aspirations.
  • Maintain accurate client service data within the Dynamics system, including case notes and files to track progress and ensure efficient record-keeping.
  • Participate in ad hoc committees to contribute to the organization's strategic goals and objectives.

Requirements and Qualifications:

  • Bachelor's degree in social work, psychology, or related field AND a minimum of 1 year of experience working with vulnerable populations OR at least 2 years of experience in providing case management to underserved communities in a similar role.
  • Self-starter, ability to work with minimal supervision; excellent communication, community relations, networking, and public speaking skills.
  • Demonstrated ability to work with families without discrimination towards people of diverse cultures, races/ethnicities, socioeconomic positions, ages, religions, genders, physical/mental challenges/disabilities, and sexual orientations.
  • Understand, explain, and apply complex local, state, and federal regulations, policies, and procedures.
  • Proficient computer skills in MS Word, Excel, PowerPoint, database management, and Internet Explorer.

Compensation:

The hourly rate is based on experience and education, along with a comprehensive benefits package including health plan/vision, dental, paid vacations, holidays, sick leave, and employer-contributed pension/group life insurance.
