Job Title: Healthcare Case Manager

Lao Family Community Development, Inc. is a community development non-profit agency established in 1980. The agency provides community development real estate facilities and a diverse array of workforce, education, and human services that directly support low-income US-born high-barrier families and individuals, refugees, immigrants, transitional-age youth, seniors, and other special populations.

This position will case manage students enrolled in a healthcare-focused employment training program, develop and cultivate employer relationships in the healthcare industry, participate in the development of a portfolio of employers with job opportunities in the healthcare field, and perform related duties as required.

Key Responsibilities:
  • Conduct individual assessments to develop appropriate personalized employment action plans.
  • Facilitate job, education, training, and other referrals to assist clients in fulfilling their employment plans.
  • Assist clients in completing applications, composing resumes, and pre-employment testing.
  • Collaborate with community organizations to provide clients with necessary clothing, shoes, and haircuts.
  • Network with healthcare-related businesses, hospitals, and clinics to identify employment opportunities.
  • Organize and conduct employment-related workshops, and job fairs, attend community outreach meetings, and speak at community group meetings.
  • Develop and maintain detailed knowledge of the local healthcare labor market.
  • Create and maintain case files; maintain accurate documentation regarding all information received and actions taken.
  • Maintain accurate logs and compile records of work performed.
  • Provide administrative and programmatic support to the Director, Managers, and/or Site Supervisors as needed.
  • Receive, sort, analyze, and summarize material for the preparation of case files; reports; and maintenance of records.
  • Prepare advanced word processing, spreadsheet, and presentation documents as assigned including manuals, brochures, reports, and PowerPoint presentations.
  • Transport clients to and from employment-related activities.
  • Be actively involved in the Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation compliance and help accomplish the goals and objectives of the agency's strategic plan.

Requirements and Qualifications:

  • Bachelor's Degree highly desired; concentration in social work, psychology, communications, or other related fields. If no degree, a minimum of 1 year of case management experience working with sensitive needs populations including but not limited to low-income, unhoused, unemployed, youth, English as a second language, etc.
  • Self-starter, ability to work with minimal supervision; excellent communication, multi-task, community relations, networking, and public speaking skills.
  • Demonstrated ability to work with families, women, and children without discrimination towards people of diverse cultures, races/ethnicities, socioeconomic positions, ages, religions, genders, physical/mental challenges/disabilities, and sexual orientations.
  • Understand, explain, and apply complex local, state, and federal regulations, policies, and procedures.
  • Able to travel to meet with service providers and participants; ability to work independently as well as part of a team; must have a flexible schedule on some evenings and weekends.
  • Proficient computer skills in MS Word, Excel, PowerPoint, database management, and Internet Explorer.
  • Must have a valid California Driver's License, clean DMV record, and proof of current auto insurance and registration; must pass a background check.

Compensation: The hourly rate is based on experience and education, along with a comprehensive benefits package including health plan/vision, dental, paid vacations, holidays, sick leave, and employer-contributed pension/group life insurance.
