Job Title: Case Manager

We are seeking a skilled Case Manager to join our team at The Salvation Army USA Western Territory. As a Case Manager, you will play a vital role in providing support and guidance to shelter residents who require assistance from our organization.

Key Responsibilities:
  • Conduct interviews with clients to assess their needs and develop personalized service plans.
  • Secure information about clients' medical, psychological, and social factors to inform case management decisions.
  • Advise clients on plans for meeting their needs and aid them in mobilizing internal capacities and environmental resources to improve social functioning.
  • Refer clients to community resources and other organizations to ensure comprehensive support.
  • Compile records and prepare reports to track client progress and service delivery.
Requirements:
  • A minimum of two to four years of related experience and/or training, or an equivalent combination of education and experience.
  • Familiarity with the social service aspects of The Salvation Army.
  • Reliable transportation and a valid California Class C Driver License.
  • Ability to lift up to 25 pounds and operate basic office equipment.
