Job Title: Case Manager

System One is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will play a critical role in supporting individuals and families experiencing homelessness by providing case management services, connecting them with community resources, and helping them achieve self-sufficiency.

Responsibilities:
  • Conduct intake and assessment of program participants to determine eligibility and develop individualized case plans.
  • Provide case management services, including goal setting, progress monitoring, and crisis intervention.
  • Connect participants with community resources, such as housing, employment, and social services.
  • Document participant progress and maintain accurate records.
  • Collaborate with internal and external stakeholders to ensure seamless service delivery.
Requirements:
  • Bachelor's degree in social work, psychology, or a related field.
  • Experience working with vulnerable populations, including homeless individuals and families.
  • Knowledge of community resources and social services.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
