Role Overview: As a Resident Care Specialist, you will play a crucial role in enhancing the quality of life for our residents by providing essential support and resources.

Key Responsibilities:

  • Assist residents and their families in navigating social services and resolving housing-related challenges.
  • Conduct regular evaluations of residents' social, psychological, cognitive, and physical well-being, and create tailored plans to meet their needs.
  • Advocate for residents to ensure they receive timely and cost-effective services.
  • Engage in initiatives that integrate housing and healthcare services.
  • Respond to emergency calls from residents in collaboration with fellow staff members during work hours.
  • Educate and guide residents and their families on available services and resources within 2Life Communities and the broader community.
  • Explore additional service options through various funding sources, including private, local, state, and federal programs.
  • Maintain accurate and confidential records of all significant interactions with residents, service providers, and families for reporting purposes.
  • Coordinate care among medical providers and facilitate safe transitions during discharge planning.

Qualifications:

  • A Master's or Bachelor's Degree in Social Work, Gerontology, or a related field.
  • Certification as a Resident Services Coordinator or a commitment to obtain this certification within the first year of employment.
  • Experience working with older adults is preferred.
  • Proficiency in Microsoft Office and database management.
  • Bilingual proficiency in Russian is an asset.
