About the Role:

Interborough Developmental and Consultation Center is seeking a highly skilled Care Coordinator/Care Manager to join its expanding Adult Health Home Plus population. As a key member of the interdisciplinary care team, you will play a vital role in ensuring that individuals with complex needs receive the necessary care and support to maintain their independence and quality of life.

Responsibilities:

  • Conduct home visits, field visits, phone, and mail outreach to individuals referred for enrollment.
  • Review client intake assessments and use results to coordinate the completion of care plans, self-management goals, and strategies; document them in the electronic medical record.
  • Travel to and visit consumers' residences or apartments to provide counsel and assistance to help consumers maintain the greatest degree of independent living.
  • Work closely with the interdisciplinary care team, including primary care physicians, psychiatrists, therapists, residential services, substance abuse treatment programs, and other stakeholders.
  • Provide input to providers, clients, and families for written individualized care plans.
  • Attend care coordination meetings, collaborate with outreach staff and external care coordinators, assess consumer needs, and discuss/advocate for necessary services.

Requirements:

  • Bachelor's degree required; three years of experience or two years of experience as a Health Home care manager in the SMI or SED population.
  • Credentialed Alcoholism and Substance Abuse Counselor (CASAC) with two years of experience.
  • Strong written and verbal skills.
  • Excellent computer skills, demonstrated ability to use word-processing, or database programs.
  • Knowledge of community resources and entitlements/benefits.
  • Good organizational and interpersonal skills.
  • Bilingual Spanish/Russian encouraged to apply.
