Job SummaryWe are seeking a highly skilled Nurse Practitioner - Advanced Practice Provider to join our team. As a key member of our care team, you will provide exceptional primary care to our patients, conducting office visits, administering wellness visits, and coordinating care with other providers and specialists.
Key Responsibilities- Provide exceptional primary care to patients, including routine and acute visits
- Administer annual wellness visits and health risk assessments, requiring a holistic view of health and thoughtful documentation
- Care coordination with other providers, specialists, testing facilities, and agencies
- Population health leadership, ensuring patients receive evidence-based care and screenings
- Assist the care team with phone triage and outreach, educating patients on their health conditions and care plans
- Participate in Oak Street Health promotional activities and conduct home visits as needed
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in a relevant specialty, such as Family Nurse Practitioner or Adult-Gerontology Primary Care Nurse Practitioner
- Active, non-probationary state Nurse Practitioner license and DEA license
- US Work Authorization
- Strong passion for teamwork and collaboration, with a desire to be part of an innovative model focused on empirically-guided population health
