Job Summary

We are seeking a highly skilled and compassionate Clinical Care Coordinator to join our team at Select Specialty Hospital - Columbus South. As a key member of our healthcare team, you will be responsible for providing exceptional patient care and ensuring seamless transitions between hospital and home.

Key Responsibilities
  • Develop and Implement Patient-Centered Care Plans: Collaborate with interdisciplinary teams to create personalized care plans that address the unique needs and goals of each patient.
  • Coordinate Discharge Planning: Work closely with patients, families, and healthcare providers to ensure timely and safe discharges, while also identifying and addressing any potential barriers to care.
  • Manage Utilization Reviews and Resource Allocation: Conduct thorough reviews of patient records to ensure accurate and efficient use of hospital resources, while also identifying opportunities for cost savings and quality improvement.
  • Facilitate Interdisciplinary Team Meetings: Lead and participate in regular team meetings to discuss patient progress, address concerns, and develop strategies for improving patient outcomes.
  • Maintain Accurate and Comprehensive Medical Records: Ensure that all patient records are up-to-date, accurate, and compliant with regulatory requirements.
Requirements
  • Current Licensure in a Clinical Discipline: Possess a current license in a clinical discipline, such as nursing, respiratory therapy, or medical social work.
  • Previous Experience in Critical Care: Have a minimum of 2 years of experience working in critical care settings, with a strong understanding of the complexities and challenges associated with this type of care.
  • Excellent Communication and Interpersonal Skills: Possess excellent verbal and written communication skills, as well as the ability to build strong relationships with patients, families, and healthcare providers.
