Role Responsibilities:

The Registered Nurse Care Navigator plays a vital role in managing and coordinating patient care. This position involves utilizing the Nursing Process to ensure adherence to organizational standards and guidelines. The RN Care Navigator collaborates with patients, healthcare providers, and insurance plans to facilitate effective care delivery within a Patient-Centered Medical Home framework.

Key Duties Include:
  • Coordinating daily nursing care for patients with chronic conditions, promoting best practices and quality initiatives.
  • Assisting physicians with patient evaluations and treatment procedures.
  • Conducting pre-visit assessments and documenting patient interactions in accordance with professional standards.
  • Developing and documenting individualized care plans while meeting accreditation requirements.
  • Providing thorough education to patients and their families regarding health conditions and treatment options.
Qualifications:
  • Graduate from an accredited nursing program.
  • Valid Registered Nurse license in the State of Ohio.
  • Minimum of two years of clinical nursing experience in a hospital setting.
  • Preferred experience in an outpatient environment.
  • Case Management experience is a plus.
