Job Overview:
Care Partners, a leading not-for-profit hospice in Oregon, seeks an experienced Licensed Practical Nurse (LPN) to join our team as a full-time or part-time LPN Case Manager. This role provides opportunities for professional growth and development in a dynamic and compassionate work environment.

About the Role:
This position is responsible for providing direct patient care, assessing, evaluating, and reevaluating patients' needs, and coordinating care with other healthcare professionals. The successful candidate will possess strong critical thinking, patient assessment, and problem-solving skills, as well as excellent verbal and written communication skills.

Key Responsibilities:
* Assesses and evaluates patients' needs, organizing resources to meet those needs
* Coordinates care provided by all hospice disciplines
* Provides nursing care as needed, attending to physical, emotional, and spiritual needs of patients
* Counsels patients and families regarding symptom control, medications, direct nursing care measures, nutrition, etc.
* Assists patients and families in making choices that best meet their needs by providing alternatives and support
* Attends deaths, supports families, assists with final arrangements
* Admits new patients, educates families, arranges services and equipment, obtains physician orders, and procures medications
* Collaborates professionally with other healthcare providers to ensure optimal care for patients
* Develops and revises patient care plans in collaboration with other IDT members, completing necessary documentation and attending IDT meetings for assigned patients

Qualifications:
* Current unencumbered Oregon licensure as a Licensed Practical Nurse or Licensed Vocational Nurse prior to first day of employment
* Prefer candidates with one year of professional LPN experience in hospice, palliative care, home health, medical surgical care, or equivalent
* Must have a valid Oregon driver's license and acceptable driving record, with access to a reliable automobile to travel to and from patient homes
* Requires well-developed critical thinking, patient assessment, and problem-solving skills, as well as excellent verbal and written communication skills
* Ability to lift 50lbs, stand and/or sit for long periods, climb stairs, and provide services in a patient's home environment

Salary and Benefits:
The estimated salary for this position is $55,000-$65,000 per year, depending on experience.
