About the Job

We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Oregon. As an LPN, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

Job Summary

The ideal candidate will possess a strong foundation in nursing principles and practices, with a focus on providing high-quality care to patients in their homes. A valid driver's license and auto insurance are also required for this role.

Key Responsibilities

  • Develop and implement individualized care plans that meet patients' unique needs
  • Collaborate with healthcare professionals to ensure seamless care transitions
  • Document patient interactions and progress accurately and thoroughly
Salary and Benefits

The estimated salary for this position is $70,000 - $80,000 per year, depending on experience. Additionally, Providence offers a competitive compensation package, including medical, dental, and vision coverage, retirement savings plan, and paid time off.

Requirements
  • Graduate degree from an accredited LPN/LVN school of nursing
  • Licensed as a Practical Nurse in Oregon
  • Valid driver's license and auto insurance
  • At least one year of experience in nursing, preferably in home health or hospice care
