Licensed Practical Nurse

Care Partners is seeking a compassionate and skilled LPN to join our interdisciplinary team as a Hospice Case Manager. This is a unique opportunity to make a meaningful difference in the lives of our patients and their families, while working in a collaborative and supportive environment.
You will be responsible for assessing patient needs, coordinating care, and providing direct nursing services to promote comfort and well-being. Our team is dedicated to providing comprehensive care that meets the physical, emotional, and spiritual needs of our patients and their families.
We offer a competitive salary range of $ $65,000 - $85,000 per year, depending on experience, as well as a comprehensive benefits package that includes low-cost medical and dental insurance, 100% paid STD/LTD/AD&D insurance, and a generous PTO program.
Main ResponsibilitiesAs a Care Partners LPN, you will be responsible for:
- Assessing patient needs and developing personalized care plans.
- Coordinating care provided by all disciplines, ensuring seamless transitions and effective communication.
- Providing direct nursing services, including symptom management, medication administration, and emotional support.
- Collaborating with the interdisciplinary team to ensure comprehensive care and address any concerns or issues that may arise.
- Communicating effectively with patients, families, and healthcare providers to ensure timely and accurate information exchange.
To be successful in this role, you will need:
- A current unencumbered Oregon licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- At least one year of professional LPN experience in hospice, palliative care, home health, or medical-surgical care.
- A valid Oregon driver's license and a reliable automobile for travel to patient homes.
- Well-developed critical thinking, patient assessment, and problem-solving skills.
- The ability to work independently and as part of an interdisciplinary team, with strong verbal and written communication skills.
- Basic life support CPR certification or the ability to obtain it within two weeks of hire.
