Job Summary

We are seeking a highly skilled and compassionate Care Coordinator to join our team at the Kewa Pueblo Health Corporation (KPHC). As a Care Coordinator, you will play a vital role in ensuring the health and well-being of our patients by providing exceptional care coordination services.

Key Responsibilities
  • Advocacy and Support: Serve as a patient advocate, providing emotional support and guidance to patients and their families, while ensuring their needs are met and their rights are respected.
  • Care Planning: Develop and implement comprehensive care plans in collaboration with patients, families, healthcare providers, and community resources.
  • Communication and Coordination: Facilitate effective communication and coordination among patients, families, healthcare providers, and community resources to ensure seamless care transitions and optimal health outcomes.
  • Community Resource Navigation: Connect patients and families with community resources, services, and support to address their social, emotional, and cultural needs.
  • Documentation and Reporting: Maintain accurate and timely documentation of patient interactions, care plans, and outcomes, while reporting on program metrics and performance indicators.
Requirements
  • Education: Bachelor's degree in a health or social sciences field from an accredited institution of higher learning, or Associate's degree with at least four years of experience in a health or social sciences field.
  • Experience: Three to four years of related work experience, preferably in a clinical setting.
  • License/Certifications: Basic Life Support (BLS) and CPR/First Aid Certifications preferred.
  • Skills and Abilities: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations, including patients, families, healthcare providers, and community resources.
Preferred Qualifications
  • Bilingual Skills: Bilingual skills in English and the Keres native language.
  • Prior Experience: Prior experience working with Indian Health Services (IHS), a Tribe or Tribal organization, or in a healthcare setting.
Work Environment

Travel during the day time, as well as limited overnight travel may be required from time to time.
