Role Objective:

The Patient Care Liaison, under the supervision of the Behavioral Health Director or designated personnel, works collaboratively with patients facing chronic illnesses or those at high risk, their families, clinical teams, and community resources to ensure comprehensive care in accordance with established regulations and standards.

Core Duties and Responsibilities:

  • Serve as a primary contact, advocate, and resource for patients, care teams, families, payers, and community organizations.
  • Conduct home visits and outreach initiatives for patients and potential participants.
  • Execute thorough assessments for program participants.
  • Engage in interdisciplinary teamwork for the care of program participants.
  • Collaborate with Managed Care Organizations regarding grievances, appeals, and critical incidents.
  • Facilitate patient access to medical and specialty services.
  • Educate patients and families about available community resources.
  • Coordinate meetings among patients, families, care teams, payers, and community resources.
  • Support co-management between primary care and specialty providers.
  • Identify high-risk patients and actively participate in program meetings.
  • Contribute to the care coordination team, focusing on various factors to enhance healthcare delivery.
  • Perform additional assigned duties as necessary.

Required Qualifications:

Education:

  • Bachelor's Degree in a health or social sciences discipline with a minimum of three years of relevant experience (preferred); or an Associate's Degree in health or social sciences with at least four years of experience (accepted).

Experience:

  • Three to four years of relevant experience in a clinical environment.

Licenses/Certifications:

  • BLS and CPR/First Aid certifications are preferred.

Essential Knowledge, Skills, and Abilities:

  • Proficient in computer applications and general office tasks.
  • Strong communication skills with a patient-centered approach.
  • Knowledge of patient rights, confidentiality, and HIPAA regulations.
  • Experience advocating for patients.
  • Commitment to ongoing professional development.
  • Proficiency in electronic health records usage.
  • Understanding of medical terminology and procedures.
  • Positive employment history.

Preferred Qualifications:

  • Bilingual in English and Keres native language.
  • BLS and CPR/First Aid certifications.
  • Experience with Indian Health Services or Tribal organizations.
  • Prior experience in a healthcare setting.

Work Environment:

This position involves working within a healthcare facility under typical office conditions. Some travel may be necessary, and health risks may be present due to the nature of the work.

Physical Requirements:

The role requires standing, walking, sitting, and engaging in various physical activities. Vision and hearing capabilities are essential for effective communication.

Mental Requirements:

This position necessitates meeting deadlines and managing complex issues while interacting with a diverse group of individuals.
