Job Summary:

We are seeking a highly motivated and compassionate individual to join our team as a Peer Navigator, Street Medicine. In this role, you will work closely with our healthcare team to provide support and guidance to patients experiencing homelessness, helping them navigate the healthcare system and access essential services.

Key Responsibilities:
  • Build rapport with patients and establish trust to facilitate effective communication and care coordination.
  • Collaborate with healthcare providers to develop and implement personalized care plans that address patients' unique needs and goals.
  • Provide education and support to patients on healthy behaviors, disease management, and wellness strategies.
  • Link patients to community resources, including housing, employment, and social services.
  • Conduct regular outreach and follow-up with patients to ensure continuity of care and address any concerns or issues that may arise.
  • Participate in interagency collaborations to promote coordination of care and improve health outcomes for patients.
  • Document patient interactions, progress, and outcomes in accordance with organizational policies and procedures.
Requirements:
  • High school diploma or equivalent required; bachelor's degree preferred.
  • 3+ years of experience working with homeless populations, preferably in a healthcare setting.
  • Strong communication and interpersonal skills, with ability to build rapport with patients and healthcare providers.
  • Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
  • Proficiency in operating a computer and navigating electronic health records systems.
  • Valid driver's license, reliable transportation, and clean driving record required.
