We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team as a Care Coordination Specialist. As a key member of our Senior Community Care (SCC) product, you will play a vital role in providing care to patients at home, in nursing homes, and assisted living facilities.

Key Responsibilities:
  • Primary Care Delivery: Deliver high-quality, cost-effective care to assigned members, managing both medical and behavioral, chronic and acute conditions effectively.
  • Care Coordination: Assess the medical necessity and effectiveness of ancillary services, coordinate care as members transition through different levels of care, and monitor the needs of members and families.
  • Program Enhancement: Regularly communicate with internal and external parties, promote the CCM program in assigned facilities, and develop and maintain positive customer relationships.
Requirements:
  • Certified Nurse Practitioner through a national board, with preferred certification as ANP, FNP, or GNP.
  • Active and unrestricted license in the state of practice.
  • Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
  • Access to reliable transportation and ability to travel to client and/or patient sites.
  • Ability to lift a 30-pound bag and navigate stairs and various dwelling conditions.
Preferred Qualifications:
  • 1+ years of hands-on post-grad experience within Long Term Care.
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
  • Proficient computer skills, including documentation of medical information with written and electronic medical records.
