We are seeking a skilled Care Coordinator to join our team at Quanacut House. As a key member of our residential program, you will provide case coordination to clients, families, and collaterals, advocating for their needs and navigating human services and other departments.
Key Responsibilities- Collaborate with multiple providers, including DCYF, schools, medical providers, and other service providers, to ensure the delivery of services to clients and families.
- Monitor residential programs and maintain Trauma Systems Therapy (TST) in the milieu setting.
- Develop and implement case plans, ensuring the needs of clients and families are met.
- Provide support and guidance to clients and families, promoting their well-being and independence.
- Bachelor's degree in a social work-related field or significant residential/childcare experience.
- Excellent communication and interpersonal skills.
- Ability to work flexible hours, including evenings and weekends.
- Knowledge of cultural, ethnic, racial, and socioeconomic issues.
- Current driver's license and reliable transportation.
