Job Summary

We are seeking a highly skilled and detail-oriented Radiology Care Coordination Specialist to join our team at Trinity Health Corporation. The successful candidate will be responsible for coordinating patient care and ensuring seamless transitions between healthcare providers.

Key Responsibilities
  • Coordinate patient scheduling and referrals to specialty providers within the healthcare system.
  • Prepare patients for appointments, providing clear and concise information and written follow-up as needed.
  • Act as a direct contact and resource for patients and families, providing emotional support and education.
  • Review radiology reports and compile information to support care pathways and algorithms.
  • Collaborate with program stakeholders to support data collection and quality reporting.
  • Participate in performance improvement activities to enhance program efficiency and patient outcomes.
  • Maintain confidentiality and handle sensitive patient information with discretion.
  • Assume responsibility for personal safety and that of coworkers, reporting hazards and unsafe practices to management.
Requirements
  • High school diploma or equivalent required; associate degree preferred.
  • At least one year of clinical oncology healthcare experience.
  • Demonstrated interpersonal skills and ability to interact with patients and medical staff.
  • Comprehensive understanding of medical terminology.
  • Ability to function autonomously and make sound judgments.
  • Proficiency in computer databases and software, including Excel and Word.
  • Ability to problem-solve and communicate effectively with internal and external customers.
Working Conditions

The successful candidate will be able to adapt to changing work priorities and balance multiple responsibilities. The position requires good manual dexterity, hearing, and communication skills.
