We are seeking a skilled and compassionate Registered Nurse to join our Home Care team as a Part-Time Weekend Registered Nurse, RN Home Care Specialist. As a Home Care RN, you will provide primary nursing care to patients in their homes, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care.
Key Responsibilities- Provide one-to-one care to patients in their homes, assessing their needs and developing individualized care plans.
- Collaborate with our Population Health Case Manager to ensure seamless care coordination and effective communication with patients and families.
- Supervise and support Home Healthcare Aides and LPNs as needed, promoting a culture of teamwork and excellence.
- Deliver therapeutic interventions, manage patient care, and achieve top decile client outcomes and patient satisfaction.
- Graduate of an approved Nursing education program, BSN preferred.
- Current Registered Nurse licensure in the state of practice.
- Minimum of one (1) year professional nursing experience, with home healthcare experience preferred.
- Strong computer and technology skills required.
- Current Driver's license and reliable transportation.
- Ability to consistently demonstrate commitment to our mission and organizational code of ethics, and adhere to our Compliance Program.
