Cambridge Health Alliance (CHA) is seeking a full-time Clinical Specialist II, Nurse Practitioner, to join our CHA PACE Program. As a member of our interdisciplinary team, you will work to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services for adults 55 years and older with chronic medical and/or mental health difficulties.

The ideal candidate will possess strong assessment and evaluation skills, with the ability to diagnose health and development problems and develop effective therapeutic plans. You will work closely with patients, families, and community members to assess health needs and develop programs to meet those needs.

Key responsibilities include:

  • Assessing the physical and psychosocial health of individuals and families
  • Evaluating and interpreting developmental status to plan and execute proper intervention
  • Diagnosing health and development problems
  • Caring for patients with acute and chronic diseases
  • Initiating, evaluating, and modifying therapeutic plans
  • Working with the community to assess health needs and develop programs

Requirements include a graduate degree from an accredited Nurse Practitioner program, current Massachusetts licensure, and certification in Psychiatric Mental Health Nursing. Cambridge Health Alliance is an equal opportunity employer and welcomes applications from diverse candidates.
