Job Title: RN Case Manager

We are seeking a skilled Registered Nurse to join our team as a Case Manager. This is a travel assignment opportunity that will allow you to work in a new location and gain valuable experience.

Job Summary:

The successful candidate will be responsible for managing patient care and coordinating with healthcare teams to ensure the best possible outcomes. This role requires strong communication and organizational skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Manage patient care and coordinate with healthcare teams
  • Develop and implement individualized care plans
  • Communicate effectively with patients, families, and healthcare providers
  • Collaborate with interdisciplinary teams to ensure seamless care transitions
  • Monitor and evaluate patient progress, making adjustments to care plans as needed
Requirements:
  • Current RN licensure in the state of New Hampshire
  • At least 2 years of recent experience in specialty applying for
  • American Heart Association BLS Certification
  • Ability to work independently and as part of a team
  • Strong communication and organizational skills
