Job Summary

The Registered Nurse in Home and Community-Based Services is responsible for delivering and overseeing nursing and personal care to patients in their residences, as directed by a physician or authorized by state/program regulations, while adhering to applicable laws, regulations, and agency policies. The RN collaborates with other healthcare professionals to coordinate the care plan and ensures continuity of care by communicating effectively with physicians, agency staff, patients, families, case managers, and community resources.

Key Responsibilities

  • Deliver high-quality clinical services within the scope of practice and infection control standards, in line with the care/service plan, and in collaboration with the patient's care team from admission to discharge.
  • Conduct clinical nursing assessments in compliance with federal and/or state program requirements and payer specifications.
  • Verify patient eligibility and medical necessity for services as defined by the payer source and agency policy.
  • Develop and update individualized care plans according to federal and/or state program requirements in conjunction with other community providers.
  • Ensure the frequency and duration of the care plan meet patient needs and initiate revisions as necessary with physician approval, in accordance with federal, state, or local regulations.

Desired Experience

  • A minimum of one year of experience as a Registered Nurse is preferred.
  • Participation in an On-Call Rotation is required.

Licensure Requirements

  • Must possess a current RN license in the state of practice.
  • Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.
  • Current CPR certification is mandatory.
