Classification: Non-Exempt/Hourly
Synopsis: The Patient Care Coordinator I (PCCI) collaborate with providers, nurses, and other clinical staff to provide comprehensive, high-quality medical care to La Clinica del Pueblo's patients. This role supports the standardization and improvement of clinic flow and efficiency while prioritizing patient needs and enhancing communication within care teams. The PCCI performs medical assistant responsibilities, administrative tasks, and ensures patient adherence to recommended health maintenance and treatment guidelines. The primary goal is to provide quality care, increase patient satisfaction, and contribute to a professional, collaborative work environment.
Qualifications
Required Education and Experience
- High School Diploma or General Education Development (GED)
- Certified or Registered Medical Assistant – required within 90 days of employment.
- Bilingual in English and Spanish
- Minimum of one year (1) year of experience as a Medical Assistant working in a clinical environment
Preferred Education and Experience
- Ability to deliver exceptional external and internal customer service and compassionate care to our patients and community.
- Working effectively with the Latino community, underserved population, HIV +, and people experiencing homelessness
- Pediatric, adult and geriatric phlebotomy, laboratory experience
Required Skills/Abilities/Certifications/Licenses
- Electronic Medical Record (EMR); eClinicalWorks is a plus.
- Ability to maintain quality, safety, and infection control standards.
- Communication skills, including knowledge of appropriate medical terminology.
- CPR or BLS certification – required within 90 days of employment.
- Travel to our other clinical facility based on operational needs.

Duties and Responsibilities
Administrative
- Pre-Visit Planning: Review patients' charts before clinic sessions to identify pending health maintenance and treatment issues. Follow up on missing referral documentation, consults, laboratory work, and diagnostic imaging and other outside patient health records.
- Arrange and ensure follow-up for internal and external referral appointments and specialty care for patients.
- Daily contact with patients, providers, nursing staff, family members, other employees, and visitors. Frequent contact with contracted services staff and referral sources.
- Communicate inventory needs for medical and office supplies to the supervisor.
- Collaborate with teams on all administrative duties to ensure task completion and continuity of care.
- Check voicemails and PCC inboxes, respond to calls assigned to the team daily.
- Provide patient education on treatment adherence, STD and HIV prevention, family planning, healthy habits, and other relevant topics.
- Support clinical functions by conducting necessary chart reviews for reports.
- Adhere to the PCC - Floater Role protocol when assigned to tasks.
- Document all follow-up activities and interactions in the patient's medical record.
- Regularly participate in one-on-one meetings with the supervisor, team huddles, triads, case review meetings, and staff meetings.
- Document member, provider, and customer feedback on areas of improvement. Resolve issues when possible or escalate to the manager, supervisor, or designated staff.
- Foster a professional, collaborative, and positive work environment with all members of the patient care team.
- Adhere to all La Clinica del Pueblo's policies and procedures.
Clinical
- Coordinate and support in managing the clinic flow.
- Provides patient education, obtains informed consent from patients, and provides pre- and post-care instructions. Prepares and assists providers during procedures.
- Administer vaccines, non-intravenous injections, and perform venipunctures as directed by a licensed provider.
- Perform on-site laboratory services and assist with dressing changes, urine dipsticks, finger/heel sticks, A1C, EKGs, adult/pediatric immunizations, injections, and venipuncture as ordered by a provider.
- Operate medical equipment in the office; EKG, autoclave, point-of-care according to established policies and procedures.
- Collects and records data regarding vital signs (pulse, temperature, respirations, blood pressure, weight, height, etc.)
- Assists in implementing the plan of care by documenting chief complaints, reviewing and reconciling patients’ medications, PHQ9, tobacco screening, reproductive life health, collecting specimens, administering treatments and injections, assisting with procedures and GYN exams, recording changes in health status, and assisting clinicians and nurses in the delivery of care.
- Dispose of biological waste, needles, and related hazardous materials according to laboratory safety and infection control practices, federal, state, and local laws, and regulations.
- Maintain a clean, adequately stocked, and safe work environment.
- Adhere to all safety protocols, including standard precautions, transmission-based precautions, and other infection control policies.
- Decontaminate, clean, package, sterilize, store, and distribute medical/surgical equipment and supplies following CDC, OSHA, and CLIA guidelines. Maintain the integrity of sterile packages.
- Support orientation and practical training of new hires.
- Schedule patient appointments in the electronic scheduling system.
- Participates in quality and process improvement initiatives to provide excellent quality of care to patients and efficient operations.
- Participates and represents LCDP’s management team at external meetings, as assigned.
- Complies and stays current with all mandatory training.
- Perform other related duties as assigned.
Supervisory Responsibilities: None
Physical Requirements: Prolonged periods of screen time, sitting at a desk, and standing.
