Pediatric Home Health Care Nurse

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality nursing care to patients in their homes. As a key member of our team, you will be responsible for delivering patient-centered care, collaborating with healthcare professionals, and promoting a positive patient experience.
Key Responsibilities- Provide one-to-one patient care in a home environment, adhering to individualized care plans and physician orders.
- Execute and document care provided, ensuring accurate and timely recording of patient progress.
- Administer prescribed medication, treatments, and therapies, while maintaining a safe and infection-free environment.
- Conduct patient assessments, coordinate care, and educate patients and families on disease management and self-care techniques.
- Participate in ongoing education and training to maintain knowledge of competencies related to the nursing profession.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Ability to speak, write, read, and understand English.
- Ability to travel.
- Ability to lift 50 pounds.
- Ability to sufficiently reposition patients and move equipment without assistance.
- Ability to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
