Job Summary

Aveanna Healthcare is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to provide high-quality patient care in the home setting. As a key member of our team, you will be responsible for delivering and documenting skilled nursing care, under the supervision of a Registered Nurse, in accordance with the developed care plan and physicians orders.

Key Responsibilities
  • Deliver and document skilled nursing care to patients in their homes, ensuring a high level of patient satisfaction and quality of care.
  • Conduct thorough patient assessments, identify care needs, and develop and implement individualized care plans.
  • Administer medications, treatments, and therapies as prescribed by physicians, and document all care provided.
  • Collaborate with healthcare teams to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
Work Environment
  • Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.
