Aveanna Healthcare is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Skilled Nursing Care Specialist. As a key member of our healthcare team, you will be responsible for providing high-quality, patient-centered care to individuals in their homes.
Key Responsibilities- Deliver Quality Patient Care: Provide skilled nursing care to patients in accordance with their individualized care plans and physician orders.
- Assess and Document Patient Needs: Continuously observe and assess patient condition and care needs, documenting all activities, assessments, and nursing actions in a timely manner.
- Develop and Implement Care Plans: Develop, implement, and update nursing care plans to ensure optimal patient outcomes.
- Collaborate with Healthcare Team: Work collaboratively with the healthcare team to ensure seamless care coordination and communication.
- Maintain Accurate Records: Maintain accurate and up-to-date patient records, including documentation of care provided and patient progress.
- Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
- Licensure: Current, unrestricted state license as a Registered Nurse in the state of practice.
- Certification: Current CPR certification.
- Experience: Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
- Experience: Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home Health Experience: Previous experience in home health care.
- Physical Demands: Must be able to speak, write, read, and understand English. Must be able to travel. Must be able to lift 50 pounds. Prolonged walking, standing, bending, kneeling, reaching, twisting. Must be able to sit and climb stairs. Must have visual and hearing acuity. Must have a strong sense of smell and touch.
- Environmental Demands: Must be able to function in a wide variety of environments, which may involve exposure to allergens and other various conditions. Possible exposure to blood, bodily fluids, and infectious diseases.
