Job Summary

Preferred Home Health Care & Nursing Services is seeking a highly skilled Clinical Supervisor to oversee the delivery of high-quality home health care services. As a key member of our team, you will be responsible for planning, coordinating, and evaluating home health services, ensuring that our clients receive the best possible care.

Key Responsibilities
  • Leadership and Supervision
    • Provide daily supervision of nurses and home health aides to ensure quality of service is within the required plan of care.
    • Appraise staff performance during the probationary period and annually, and provide teaching and counseling necessary to enhance the delivery of services and the professional development of nursing and home health aide staff.
  • Client Care
    • Identify and evaluate client needs and appropriateness of home care by reviewing the initial intake screening form before assignment of direct care staff.
    • Conduct client visits in the appropriate setting as needed.
  • Administrative Tasks
    • Assist with the orientation of new professional staff to familiarize them with policies and procedures.
    • Initiate in-home supervisory visits of clinical staff per state requirements.
    • Maintain confidentiality of medical records, personnel records, etc., within the scope of their position.
  • Technology and Compliance
    • Work closely with the electronic medical record (EMR) to ensure client electronic charts are maintained, updated, and remain in compliance with company standards.
    • Conduct EMR chart audits.
Requirements
  • Registered nurse in good standing, holding a Bachelor of Science degree in nursing and having had two years of combined home care nursing and progressive professional responsibilities in home care nursing; or Registered nurse in good standing, having three years of combined home care nursing and progressive professional responsibilities in home care nursing.
  • Knowledge and experience in trach, vent, g-tube, and pediatric care.
  • Ability to demonstrate self-confidence and positive attitude toward self and others, and maintain commitment and enthusiasm to achieve goals.
  • Ability to identify and evaluate personal strengths and weaknesses.
  • Customer-service oriented.
  • Working knowledge of, or proficient in, Microsoft office applications, electronic medical records, and quick-to-learn proprietary software package.
  • The ability to communicate well, both verbally and in writing.
