Job Overview

We are seeking a highly skilled Registered Nurse to join our team as a Patient Care Coordinator. This role is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work collaboratively with healthcare professionals, patients, and families. A strong understanding of medical necessity, utilization management, and case management principles is essential for success in this role.

This position requires a dynamic individual who can navigate complex healthcare systems, identify opportunities for improvement, and implement evidence-based practices to promote timely throughput, safe discharge, and prevent avoidable readmissions.

We offer a competitive salary range of $120,000 - $150,000 per year, depending on experience. Our benefits package includes comprehensive health insurance, dental and vision coverage, employee assistance programs, life insurance, and a 401(k) savings plan.

Key Responsibilities
  • Facilitate care coordination across the continuum to ensure seamless transitions between healthcare settings.
  • Develop and implement transition plans to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Collaborate with healthcare professionals, patients, and families to provide education and support.
  • Monitor and analyze data to identify trends and opportunities for improvement.
  • Communicate effectively with internal stakeholders to promote efficient and effective care delivery.
