Seeking an Advanced Medical Care Coordinator to join our team at the Detroit Medical Center (DMC), a leading academically-integrated hospital system in Metro Detroit. As a seasoned professional, you will play a vital role in coordinating patient care and services within our multidisciplinary teams.

About the Role

This is a full-time position working 36 hours per week, with a 12-hour shift on day shifts. The ideal candidate will have a strong background in medical coordination and a passion for delivering exceptional patient care.

Job Responsibilities:
  • Collaborate with healthcare teams to develop and implement comprehensive treatment plans for patients
  • Coordinate interdisciplinary team meetings to discuss patient rehabilitation programs
  • Assess patient needs and recommend services based on individual assessments
  • Participate in continuous performance improvement activities to enhance quality of care
  • Maintain effective relationships with physicians, staff members, and other stakeholders
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work, Speech Language Pathology, or a related field
  • Current licensure, registration, or certification as applicable
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
