We are seeking an experienced Transplant Coordinator to join our team at Duke Health, a leading healthcare organization dedicated to advancing health together. As a key member of our Hematologic Malignancies & Cellular Therapy team, you will play a critical role in delivering exceptional patient care and contributing to our mission of excellence in patient care, research, and teaching.
Key Responsibilities- Educate patients and families regarding all aspects of cell therapy care, including arranging follow-up appointments, labs, and working with the multidisciplinary team on patient-specific needs.
- Work closely with all HMCT faculty and staff, providing a source of truth for all commercial CAR-T products, patients, and data.
- Communicate with patients and caregivers regarding the plan of care and ongoing changes.
- Provide consistent alignment & communication with pharmaceutical vendors.
- Keep track of all patient-related care data, including visits, quality information, outcomes, etc.
- Provide for continuity of care between inpatient care, outpatient care, and long-term management.
- Ability to make clinical judgments based upon synthesis of current research, concepts, principles, theories, and outcomes to solve complex problems.
- Collaborate with medical faculty, pharmacy, nursing personnel, case management, and other health disciplines to facilitate meeting patient and family needs.
- Complete regulatory documentation for FACT and REMS training.
- Review education materials and work with the department if there are changes in current practice.
- Current or compact RN licensure in the state of North Carolina required.
- BLS required.
- Minimum 2 years clinical nursing experience.
- Preferred: 2 years nursing experience in hematologic malignancies, blood & marrow transplant, and/or cellular therapies.
