About the Role

We are seeking a dedicated and compassionate Youth Reunification Specialist to join our team as an After-Hours Bilingual Case Manager. This critical role involves providing culturally sensitive case management services to unaccompanied adolescents in foster care, aiming for their safe return and reunification with their families.

Key Responsibilities
  • Maintain a caseload of assigned youth and engage with them weekly to assess placement needs and ensure smooth transitions.
  • Collaborate with the Mental Health Therapist to develop treatment plans and goals tailored to each youth's unique needs.
  • Coordinate with foster parents to provide life skills education and support.
  • Establish aftercare treatment plans and monitor progress, ensuring timely assessments and documentation.
Requirements and Qualifications
  • Bilingual proficiency in English/Spanish is mandatory.
  • A Bachelor's Degree in behavioral sciences, human services, or social service fields is required.
  • At least 3 years of experience in child welfare, DUCO, or case management is preferred.
