Nurse Case Manager for Acute Care

We are seeking a highly skilled Nurse Case Manager to join our team at Swedish Health Services. This role is responsible for coordinating the care and service of selected patient populations through the acute care episode, across the continuum.
Responsibilities- Collaborate with inter-disciplinary teams, both internal and external to the organization, to improve patient care through effective utilization and monitoring of healthcare resources.
- Assume a leadership role to achieve desired clinical, financial, and resource outcomes.
- Facilitate the progression of care by advancing the care plan to achieve desired outcomes.
- Develop and document a discharge plan through collaboration with the interdisciplinary team.
- Ensure that all activities to facilitate and coordinate the plan are being implemented and that the plan is continuously modified based on the patient's changing needs.
- Bachelor's Degree in Graduate of an accredited Bachelors Nursing degree program or completion of an accredited Nursing program with a Bachelors degree or higher in a health care field.
- Washington Registered Nurse License upon hire.
- 3 years Registered nursing experience in an acute care hospital.
The estimated salary for this role is $57,000 - $73,000 per year, depending on experience. Additionally, Providence offers a comprehensive benefits package including health insurance, retirement savings, paid time off, and more.
