Culturally Responsive Case Coordinator

Friends of Youth is a leading organization dedicated to supporting vulnerable youth and families. We are seeking a skilled and compassionate Culturally Responsive Case Coordinator to join our team and play a key role in providing high-quality case management services.
About the Position
- Maintain direct contact with each assigned client and have at least one documented, scheduled weekly meeting with each client to discuss reunification options.
- Collaborate with the Mental Health Therapist in the development of culturally and developmentally appropriate treatment plans and treatment outcome goals for each youth.
- Assist foster parents in their role as primary life skills educators.
- Collaborate and facilitate quarterly treatment service plans, goals, modalities, and individualized approaches for each youth on their caseload.
- Contact and develop community resources to support youth's treatment plans, goals, and discharge planning.
- Establish aftercare treatment plans for all youth on caseload and monitor and implement these aftercare plans.
- Ensure completion of timely assessments as contracted.
- Maintain case notes as required, including documentation of all medical, dental, and treatment sessions and updated in the UC Portal where applicable.
- Disseminate and implement agency policies.
- Support MHT treatment methods that incorporate agency goals, policies, and procedures.
- Assist foster parents in providing transportation of youth to appointments, community resources, and meetings.
- Provide support to and collaborate with foster care team to carry out services to youth in the program.
- Assist in and attend, foster family monthly support meetings and other activities that may be scheduled.
Key Responsibilities
- Develop and maintain relationships with community partners and stakeholders to ensure effective delivery of services.
- Coordinate and participate in case planning and treatment teams to ensure comprehensive and culturally responsive services.
- Monitor and report on client outcomes and program effectiveness.
- Provide education and training to staff and stakeholders on cultural competence and trauma-informed practices.
Requirements
- Bilingual in English/Spanish.
- Bachelor's degree in behavioral sciences, human services, or social service fields.
- 3 years of progressive employment experience in the aforementioned fields is preferred.
- Child welfare, DUCO, and/or case management experience is strongly preferred.
Salary Range
The estimated salary for this role is $60,000 - $70,000 per year, depending on experience.
