Compensation Package

We offer a competitive salary of $85,000 per year, which is reflective of your experience and qualifications as a RN Case Manager. In addition to your base salary, you will also be eligible for a range of benefits, including medical, dental, and vision coverage, as well as paid time off and holidays.

About the Role

As a RN Case Manager, you will be responsible for providing comprehensive care to our patients, working closely with their healthcare teams to develop and implement personalized care plans. You will be expected to assess patient needs, coordinate care services, and ensure that all necessary interventions are implemented in a timely and effective manner.

You will work collaboratively with other healthcare professionals to ensure seamless transitions of care, and will maintain accurate records and reports to document patient progress and outcomes.

Requirements
  • Graduate from an approved school of professional nursing and currently licensed to practice as a registered nurse in the state of agency operation.
  • One (1) year experience as a RN.
  • Valid CPR certification while employed in a clinical role.
