Job Summary

This role is responsible for providing diabetes education services to patients and families in the Diabetes or Obstetric Programs. The successful candidate will collaborate with clinical care teams to develop and implement diabetes management plans, provide education and support to patients and families, and facilitate diabetes medication prescription renewals.

Key Responsibilities
  • Provide diabetes education classes and support group services to patients and families
  • Collaborate with clinical care teams to develop and implement diabetes management plans
  • Facilitate diabetes medication prescription renewals and assist with prior authorizations for diabetes medications and devices
  • Monitor patients' diabetes behavioral goals and outcomes, and communicate and document patient progress for the healthcare team
  • Assist in-patient hospital staff with diabetes management skills and education
Requirements

The ideal candidate will have a Bachelor's degree in a work-related discipline/field and one year of progressively responsible and directly related experience. Certification as a Certified Diabetes Care and Education Specialist (CDCES) is also required.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to adjust communications to fit the needs and level of understanding of the receiver. They will also have the ability to provide leadership and influence others, and to teach and provide instruction to patients and their families in areas related to their treatment.
