Client Services Coordinator

The Service Coordinator I will deliver comprehensive case management services, employing a recovery-oriented approach that merges mental health support, substance use services, and community resources for an assigned group of clients. Our primary focus areas include:
· Integrated Case Management: (Behavioral Health)
o Target Audience: Deliver case management to adults diagnosed with severe mental health conditions and children/adolescents experiencing mental health challenges or significant emotional disturbances.
· School-Based Case Management: (Student and Family Support)
o Target Audience: Provide case management services for students and their families within the public school system, collaborating closely with educational institutions and UPMC Western Behavioral Health.

Sign-On Bonus Details:
- $3,000 for a commitment of 18 months of service.
Key Responsibilities:
- Provide holistic case management to an assigned group of clients, utilizing a strengths-based and person-centered recovery framework.
- Exemplify the agency's mission and values in all aspects of work.
- Maintain a flexible schedule to accommodate the needs of clients and the program.
- Participate in on-call duties for after-hours support and advocacy.
- Effectively use electronic health records while adhering to HIPAA and privacy regulations.
- Document all client interactions and ensure timely updates of records.
- Conduct regular assessments to create personalized recovery plans that address various life domains, including mental health, substance use, medical, economic, housing, and vocational needs.
- Facilitate access to community resources and programs to support client needs.
- Monitor service effectiveness and explore additional resources as necessary.
- Assist clients in developing Safety Crisis/Relapse Plans, identifying triggers and helpful strategies during crises.
- Advocate for clients during hospitalization processes, including assessments, discharge planning, and aftercare referrals.
- Provide crisis intervention services as needed.
- Engage in quality assurance and improvement initiatives.
- Foster positive relationships with clients, colleagues, and community organizations to enhance service delivery.
Qualifications:
- Bachelor's Degree in a Human Services discipline such as Social Work, Psychology, or Counseling is required.
- Other relevant fields may be considered with a minimum of 12 credits in Human Services coursework.
- One year of related experience is preferred, especially in community settings.
- A valid driver's license and a safe driving record are required.
