Position Overview

Community Support Case Manager

As a vital member of the Assertive Community Treatment (ACT) team, the Community Support Case Manager plays a crucial role in advocating for and assisting assigned adult clients. The multidisciplinary team includes specialists focused on various aspects of client care, such as housing, vocational support, therapy, medical assistance, and substance use recovery.

In this role, you will collaborate with clients to develop and execute personalized service plans that reflect their self-identified goals and needs. This includes assessing their strengths, barriers, support systems, and emotional triggers. Utilizing these plans, you will connect clients with essential community resources and provide direct support and training in vital life skills, including budgeting, grocery shopping, home maintenance, meal preparation, social interaction, coping mechanisms, and self-monitoring of psychiatric symptoms.

Regular home visits will allow you to track client progress and behaviors, working alongside the ACT team to facilitate the achievement of client objectives. This position may require client transportation and includes a weekly on-call rotation every six weeks, managed virtually or in person as circumstances dictate. For optimal outcomes, client meetings will primarily occur within the local community.

About the ACT Program:

Established in the 1970s, the Assertive Community Treatment (ACT) model is one of the most extensively researched evidence-based practices for supporting adults with severe mental illnesses, particularly those aged 25 and older. By providing comprehensive, person-centered assistance—including housing, recovery, vocational support, and access to nursing, psychiatric, and therapeutic services—we empower clients to transition to lower levels of care or achieve greater independence, thereby enhancing their quality of life. Our clientele may include individuals with histories of trauma, homelessness, frequent hospitalizations, refugees, and those who have not benefited from traditional outpatient care.

Qualifications:

  • Bachelor's degree in Psychology, Social Work, or a related discipline
  • Preferred experience working with adults facing severe and persistent mental health challenges
  • Strong professional communication skills, both verbal and written, with a commitment to cultural sensitivity and awareness of diverse populations
  • Valid driver's license in Georgia with a clean driving record
  • Successful completion of a criminal background check, with no history of abuse, neglect, or exploitation
