Position: Medical Case Manager
Location: Remote
Responsibilities:
- Monitor the health status of all assigned personnel.
- Arrange for necessary evaluations and audiology assessments at designated health facilities.
- Collaborate with medical institutions to develop comprehensive treatment plans for personnel.
- Communicate with individuals to gather essential information for their case management records, ensuring all referrals from providers are addressed.
- Oversee the care of personnel from the initial identification of health issues until they are deemed medically fit for duty or separated from service.
- Provide education on available resources to enhance medical and dental readiness.
- Review and track health assessment laboratory results, advising personnel on lifestyle changes as needed.
- Update medical records as necessary, including immunizations and other health indicators.
- Coordinate medical appointments with units to ensure attendance and proper documentation follow-up.
- Prepare medical documentation for investigations related to Line of Duty (LOD) in collaboration with Health Service Support (HSS).
- Ensure timely processing of medical records for personnel transitioning to mobilization sites.
- Input individual vouchers for personnel's Individual Medical Readiness (IMR) requirements into the Automated Voucher System (AVS) when necessary.
Qualifications:
- A minimum of 5 years of experience in case management.
- At least 3 years of clinical experience is required.
Education/Certification:
- Must possess a Registered Nurse III (RN) License.
- Certification as a Certified Case Manager (CCM) is preferred.
