*Role Overview: Our Advanced Practice Providers (APPs) play a vital role in delivering compassionate care to elderly patients with complex health needs in their homes. Responsibilities include providing primary, urgent, and acute medical care, maintaining accurate patient records, and coordinating appointments as necessary. Successful candidates will collaborate with a multidisciplinary team to ensure comprehensive care for each patient, fostering a positive and collaborative work environment.

Key Responsibilities:

  • Uphold the WellBe mission of supporting patients in leading healthier, more meaningful lives through comprehensive care.
  • Collaborate with fellow clinicians to manage the health and wellness of patients in their homes.
  • Engage patients in the WellBe program to enhance their care experience.
  • Deliver primary care services directly in patients' homes.
  • Provide educational resources to facilitate informed decision-making among staff, patients, and families.
  • Conduct initial screenings and physical examinations, developing appropriate care plans.
  • Address urgent care needs as they arise.
  • Work with the community team to complete thorough care assessments.
  • Order and interpret diagnostic tests, determining follow-up needs and treatment options.
  • Prescribe and manage medications, including psychopharmacological agents as appropriate.
  • Ensure accurate and up-to-date patient records at each appointment.
  • Deliver high-quality care while adhering to company metrics for documentation and patient engagement.
  • Serve as a resource for patients, providing ongoing care information and support.
  • Refer patients to internal and external resources as needed.
  • Participate in interdisciplinary team meetings to enhance care coordination.
  • Maintain required licensure and certifications.
  • Provide palliative and end-of-life care when necessary.
  • Facilitate family meetings to address complex care dynamics.
  • Perform additional tasks as needed to achieve team objectives.

Qualifications:
*Essential Qualifications:

Education and Experience:

  • Master’s Degree in Nursing.
  • Valid nurse practitioner or physician assistant license and current board certification.
  • Minimum of 5 years of progressive care experience.
  • BLS and ACLS certification and an NPI# as a Nurse Practitioner.
  • Valid driver’s license and access to a vehicle for patient visits.

Skills and Competencies:

  • Exemplary teamwork, integrity, and productivity.
  • Proficient in diagnosing and treating chronic and urgent medical conditions in older adults.
  • Experience in geriatrics, internal medicine, and palliative care.
  • Strong communication skills, both verbal and written.
  • Ability to engage effectively with patients and families.
  • Proficient in computer applications including Word, Excel, and PowerPoint.
  • Previous home care experience is preferred.
  • Excellent time management and multitasking abilities.
*Travel Requirements: Local travel may be required up to 100%.

*Work Environment:
Ability to lift up to 20 lbs. and assist with patient transfers that may exceed 100 lbs. Ability to stand for extended periods and drive to various patient locations. Fine motor skills and visual acuity are necessary.
