Key Responsibilities- Collaborate with Interdisciplinary Team: Work closely with our team of healthcare professionals to develop and implement individualized treatment plans that meet the unique needs of each patient.
- Coordinate Patient Care: Schedule family conferences, communicate with caregivers, and provide regular updates on patient progress and treatment plans.
- Facilitate Treatment Planning: Coordinate weekly patient care team conferences to refine treatment plans and achieve identified patient goals and outcomes.
- Identify and Address Barriers to Care: Consult with other departmental staff to identify and resolve barriers to care and discharge.
- Document Patient Progress: Complete documentation per workflow timeline and content requirements, including the Individual Plan of Care (IPoC) per CMS guidelines.
- Licensure and Certification: Current Registered Nurse or Social Work licensure or Healthcare professional licensure as Respiratory Therapist, Physical Therapist, Speech Language Pathologist, or Occupational Therapist.
- Experience and Skills: Minimum of 2 years social work or case management experience in an inpatient setting, with acute/rehabilitation hospital experience preferred. Effective oral and written communication skills in English, with additional languages a plus.
- Education and Training: Basic computer skills in Excel, Word, Outlook, PowerPoint, etc. required.
We are a 34-bed inpatient acute rehabilitation hospital operated jointly with Lifepoint Health and Anderson Healthcare. Our mission is to maximize the health, function, and quality of life of those we serve through comprehensive physical medicine and rehabilitation programs. Our rehabilitation programs provide ongoing care and specialized treatment to patients throughout their recovery journey, addressing conditions such as stroke, brain injury, neurological conditions, trauma, spinal cord injury, amputation, and orthopedic injury.
