Community Liaison Specialist

We are seeking a highly motivated and compassionate Community Liaison to join our team at Community Residences. As a Community Liaison, you will play a vital role in ensuring the health, safety, and well-being of our developmentally disabled clients in our Day Program facilities.
Key Responsibilities- Client Support and Development: Work closely with individuals, their families, and their natural supports to develop opportunities for success, including assessments of strengths and interests, identification of activities that promote social growth, and expansion of natural personal supports.
- Community Engagement: Assist individuals and others in identifying future goals and develop plans to achieve them, while working in conjunction with Day Support staff to increase individual matching of personal interests and preferences to identify community resources and opportunities.
- Learning Environment and Support: Create a learning environment that promotes independent performance in daily living, social, emotional, and physical situations, and facilitate effective coping strategies.
- Individual Plan of Care: Implement and follow Individual Plans of Care, as identified by the Interdisciplinary Team, and coordinate and monitor effective supports.
- Training and Mentoring: Provide training and mentoring to new and existing employees, and maintain individuals' right to privacy and adhere to confidentiality policies.
- Physical Demands and Safety: Must be able to lift and transfer 70-75 lbs, and utilize approved protective hold techniques to ensure safety.
- Professional Development: Participate in training opportunities, maintain required training, and demonstrate organizational ability, effective communication, and supervisory skills.
- Valid Driver's License: Possess a valid RI driver's license and a good driving record.
- Certifications: Must be able to become certified in CPR and First Aid.
- Education and Experience: Bachelor's Degree, or an associate degree and one year experience in the Human Services Field, or a high school diploma or GED with two years' experience in the Human Services field.
- Motivation and Interest: Display motivation and interest in improving quality of life for the population served.
- Teamwork and Communication: Display interest in being part of a multi-disciplinary teaching approach and demonstrate ability to communicate effectively with adults who are developmentally disabled.
