This role involves the development, coordination, and delivery of social work services aimed at assisting individuals facing emotional, social, and situational challenges due to health-related issues.

The primary objective is to facilitate adaptation and empower both patients and their families to engage fully in care coordination and discharge planning.

This position requires the provision of care that is appropriate for various developmental stages, ensuring safe discharge and continuity of care while addressing the distinct needs of diverse populations, including those who are physically disabled, mentally ill, chronically ill, or terminally ill.


Compensation & Benefits:

Salary:
Competitive hourly wage ranging from $28.47 to $44.49, commensurate with experience and education.

Retention bonuses are available.

Relocation Support:
Up to $7,500 in relocation assistance.

Temporary housing for 90 days is provided.

Additional Compensation:
Shift differentials, annual salary increases, and paid time off.

Available Shifts:

  • Full-time positions, 40 hours per week, with 4X10 hour shifts.
  • Shift hours from 8:00 AM to 6:30 PM, including weekend rotations (no on-call duties).

Shift Differentials:
$2.00/hr for weekends, $2.50/hr for evenings, and $3.50/hr for night shifts.

Responsibilities:
As a Clinical Social Services Specialist, you will:
  • Manage care coordination and facilitate timely discharge or transfer of patients from acute care settings.
  • Assess and evaluate patient needs considering their functional abilities, cultural background, and available support systems.
  • Provide crisis intervention and counseling to patients and families experiencing complex or high-risk situations.
  • Collaborate with the healthcare team to implement and communicate effective care plans.
  • Document all interventions accurately and promptly in the patient medical record.
  • Advocate for patients and connect them with community resources and services.
  • Maintain up-to-date knowledge of Medicare, Medicaid, and other relevant program benefits.
  • Deliver professional education to staff and community members.

Qualifications:
Applicants must possess a Master's Degree in Social Work and obtain a non-temporary AK LMSW license within 12 months of hire. A temporary LMSW license is acceptable during the first year of employment.

Candidates should have at least one year of experience in discharge planning within a healthcare environment. Knowledge of community resources and effective communication skills are essential. Flexibility in working hours and participation in rotation on-call duties is required.

PREFERRED QUALIFICATIONS:
BLS Certification and additional relevant education or experience are preferred.
