Key Responsibilities:
- Manage individual patient care across the healthcare continuum to optimize clinical, financial, operational, and satisfaction outcomes.


- Lead process improvement initiatives for patient populations to achieve optimal results.


- Collaborate in developing and managing interdisciplinary patient discharge plans, ensuring effective communication throughout the care continuum.


- Evaluate the necessity and appropriateness of care, optimizing patient outcomes while addressing potential discharge delays.


- Foster collaborative relationships with physicians, payers, and healthcare team members, ensuring timely communication of pertinent information.


- Educate team members on case management and managed care concepts, integrating these principles into daily practices.


- May supervise other staff members.


- Adhere to established policies, procedures, and regulatory requirements while providing excellent service to all customers.


Qualifications:


- A bachelor's degree in case management or healthcare is required, along with knowledge of case management or utilization review.


- Current Registered Nurse (R.N.) license in the state of practice is mandatory. Basic Life Support (BLS) certification is required for acute care settings.


- A minimum of 5 years of clinical experience is necessary, with a solid understanding of care management, acute care, and home care environments.


- Demonstrated critical thinking, problem-solving, effective communication, and time management skills are essential.


- Ability to work effectively in an interdisciplinary team format is required.


- Flexibility to work varying hours and participate in on-call rotations after hours is expected for acute care settings.


- Experience in an acute care hospital or home care setting within the past 12 months as a Case Manager is preferred.


PREFERRED QUALIFICATIONS:
- CCM (Certified Case Manager) certification is preferred.
