Full-Time/36-hour work week/12-hour shifts

- Responsibilities: Evaluates patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Reviews patient history and physical examination with the patient/family, ensuring timely completion. Analyzes available information from other healthcare team members. Reviews diagnostic and laboratory data, reporting any abnormal results to the physician(s) and other relevant caregivers. Conducts assessments and reassessments based on patient needs and established policies.
Implements the care plan through direct patient care, coordination, delegation, and supervision of healthcare team activities.
Delegates appropriately and supervises nursing care provided by other licensed nurses and personnel.
Facilitates continuity of care by accurately communicating patient status to other caregivers.
Assesses patient and family responses to care and education, adjusting the care plan as necessary. Evaluates patient progress towards established goals and expected outcomes in collaboration with other healthcare team members. Assesses patient responses and the effectiveness of educational interventions.Documents assessment, planning, implementation, and evaluation in the patient record. Ensures documentation is clear, timely, and adheres to policy. Documentation reflects objective/subjective data, nursing interventions, and patient responses to treatment. Accurately notes physician orders in a timely manner.
Provides care based on the best available evidence and may engage in research activities within clinical practice. Participates in unit or facility shared leadership. Engages in the education, role development, and orientation of facility personnel, patients, students, families, and visitors. Supports the growth of others by serving as a preceptor or mentor when appropriate.
Contributes to societal well-being through activities that promote excellent patient outcomes via timely, effective, efficient, equitable, and safe care. Actively engages in improving national nursing quality indicators and outcomes, which may include participation in professional organizations.
In certain roles, this position may involve supervising staff and managing departmental workflow.
Performs all functions in accordance with established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
Ensures all customers of Foundation Health receive an exceptional service experience by consistently demonstrating core and leadership behaviors.
Qualifications:
Must possess a valid current or temporary nursing license. Requires knowledge and skills typically demonstrated by an Associate's degree in nursing. BLS certification is mandatory. Additional certifications such as ACLS, PALS, or continuing education may be required based on the area of practice.
Requires creative thinking, prioritization, and flexible problem-solving skills. Strong verbal and written communication abilities are essential.
PREFERRED QUALIFICATIONS
A Bachelor's degree is preferred. Professional certification is advantageous.
Additional related education and/or experience is preferred.
