COMPENSATION: $23-$28 per hour

SUPERVISION: Reports to Lead Case Manager, Director of Waiver Services

WORK ENVIRONMENT: Remote

POSITION SUMMARY

The Case Manager plays a crucial role in the coordination of care for clients enrolled in the ALWP program through the Care Coordination Agency (CCA). This role involves the identification, organization, coordination, and monitoring of client needs to ensure optimal service delivery. The Case Manager is responsible for verifying ongoing eligibility for the ALWP by conducting regular follow-up assessments.

QUALIFICATIONS
  • Bachelor's Degree in social work, psychology, counseling, rehabilitation, gerontology, or sociology, accompanied by a minimum of one year of relevant experience.
  • Current CPR certification is required.
  • Familiarity with California's Assisted Living Waiver Care Coordination Agency Provider Handbook and RCFE Provider Handbook.
  • Preferred experience in education, training, quality assurance, and management.
  • Demonstrated clinical judgment and knowledge of contemporary clinical practices, with strong analytical and documentation skills.
  • Excellent organizational abilities and self-motivation to work independently.
  • Proficient in computer usage and electronic information systems, with strong communication skills to foster relationships with colleagues.
  • Knowledge of Title 22 regulations.
  • Must possess a valid driver's license and have access to a reliable, insured vehicle.
KEY RESPONSIBILITIES
  1. Verify Medi-Cal eligibility for clients on a monthly basis.
  2. Schedule and conduct monthly client visits.
  3. Engage with clients to assess their needs, including durable medical equipment, appointments, and medication compliance.
  4. Collaborate with facility staff to ensure all necessary reports are completed and address any Medi-Cal issues.
  5. Maintain accurate documentation of visits and client interactions in the designated systems.
  6. Ensure timely upload of all notes following client visits.
  7. Monitor client transitions between facilities and report updates to the CCA Coordinator.
  8. Assist in the re-assessment process by coordinating with nursing staff and ensuring documentation is completed promptly.
  9. Adhere to confidentiality standards, HIPAA regulations, and healthcare laws.
  10. Maintain accurate timekeeping and comply with organizational policies and procedures.
  11. Participate in required training and departmental meetings.
  12. Fulfill additional responsibilities as assigned.
