Job Title: Service Coordinator

Pathways Vermont is seeking a highly skilled and compassionate Service Coordinator to join our Assertive Community Treatment (ACT) team in Chittenden County, Vermont.

About the Role:

The Service Coordinator will provide flexible, community-based services to individuals experiencing mental health challenges, histories of homelessness, and substance use issues. This role requires strong interpersonal skills, sound knowledge of supportive counseling, and the ability to form relationships with marginalized individuals.

Key Responsibilities:
  • Engage in relationship building and rapport building with clients, providing services from a client-centered, harm reduction, and strengths-based approach.
  • Assist clients with referrals to community agencies, ensuring access to necessary services and resources.
  • Provide one-to-one support and counseling on life skills, housing concerns, and health maintenance.
  • Maintain accurate records, compile reports, and complete program documentation.
  • Participate in 24/7 on-call services rotation with team members.
Requirements:
  • Excellent interpersonal and communication skills.
  • Sound knowledge of supportive counseling and ability to form relationships with marginalized individuals.
  • Excellent problem-solving skills and ability to approach challenging situations intuitively.
  • Prior experience working in mental health and/or harm reduction agencies, preferably in community services.
  • Valid driver's license, reliable personal vehicle, and current automobile insurance.
