Job Title: ACT Team Service Coordinator

Pathways Vermont is seeking a highly skilled and compassionate Service Coordinator to join our Assertive Community Treatment (ACT) team in Chittenden County. As a key member of our interdisciplinary team, you will provide flexible and community-based services to individuals experiencing mental health challenges, homelessness, and substance use issues.

Responsibilities:
  • Build strong relationships with clients and provide services from a client-centered, harm reduction, and strengths-based approach.
  • Assist clients with referrals to community agencies and ensure they have access to necessary services and resources.
  • Provide one-to-one support and counseling on life skills, housing concerns, and health maintenance.
  • Maintain accurate records and compile reports to support program documentation.
  • Participate in 24/7 on-call services rotation with team members.
  • Attend annual All Staff and Programmatic Retreats, unless excused by manager/HR.
Qualifications:
  • Excellent interpersonal and communication skills, with a strong understanding of supportive counseling principles.
  • Ability to form relationships with marginalized individuals and provide services in a culturally sensitive manner.
  • Valid driver's license, reliable personal vehicle, and current automobile insurance required.
  • Willingness to self-reflect, learn, and practice new ways of being in relationships.

  • Competitive salary range: $20-$21 per hour, with up to 8.5% increase available after 1 year of employment.
