Position Overview:

The EHG - MDS Coordinator is responsible for overseeing and managing the resident assessment process, ensuring compliance with Federal and State regulations as well as organizational policies.


Work Environment:

This role is primarily conducted indoors within long-term care facilities, encompassing various areas including resident rooms. The position involves regular interaction with colleagues, healthcare professionals, residents, and visitors.

Due to the nature of the healthcare environment, the coordinator may encounter occasional hazards such as slippery surfaces, sharp objects, and exposure to infectious materials.


Key Responsibilities:
  • Fulfill physical and sensory requirements necessary for the role.
  • Engage in initiatives aimed at enhancing customer satisfaction and operational efficiency.
  • Facilitate Daily PPS meetings, weekly Medicare discussions, and month-end reviews to ensure adherence to federal billing standards.
  • Supervise the completion of the Resident Assessment Instrument (MDS, CAA's, and Care Plan) in alignment with current regulations.
  • Collaborate with the Interdisciplinary Team to gather information for the MDS, utilizing medical records, assessments, and interviews.
  • Establish a verification system to ensure all assessments are completed accurately and in a timely manner.
  • Review and validate MDS documents prior to submission, ensuring compliance with federal guidelines.
  • Complete Care Area Assessments (CAA's) promptly, providing clear documentation of care planning decisions.
  • Develop and maintain an Interdisciplinary Plan of Care, reviewing it regularly to incorporate necessary updates.
  • Provide ongoing education regarding the RAI process to team members and caregivers.
  • Participate in special projects and additional duties as assigned.

Qualifications:

Registered Nurse with active state licensure and a minimum of three years of clinical experience, preferably in long-term care settings.

Current CPR certification is required.

Demonstrated integrity and professionalism in interactions with the Interdisciplinary Team, patients, and families.

Strong knowledge of nursing practices, regulations, and guidelines relevant to long-term care.

Self-motivated with a commitment to mastering the RAI Process to enhance patient outcomes.

Excellent analytical, organizational, and communication skills, with proficiency in Microsoft Office applications.

Ability to work independently, manage multiple projects, and maintain confidentiality.

Customer service-oriented with a strong attention to detail and the capacity to thrive under pressure.


Physical and Sensory Requirements:

Moderate physical activity is required, including the ability to lift and move a minimum of twenty-five pounds and to stand or walk for extended periods.

Tasks may involve repetitive typing and focused attention on screens or documents.
