Key Responsibilities
  • Provide personalized support to individuals with disabilities to overcome functional deficits and achieve competitive employment.
  • Deliver ancillary supports and coordinate services needed by PROS recipients based on medical need.
  • Secure referrals to ORS from case load, group leaders, and client self-report.
  • Assist individuals with job readiness, job search, and employment skills.
  • Collaborate with PROS Program Supervisor to coordinate support services and meet daily with team members.
  • Provide ongoing rehabilitation services, intensive rehabilitation services, or community rehabilitation services as per individual's plan.
  • Report and record daily and bi-monthly notes as per PROS regulation and maintain files, evaluates, and modifies as necessary.
  • Facilitate group activities with PROS participants for CRS service categories.
  • Meet individually with PROS participants to provide ORS services.
  • Develop IAP for individuals receiving PROS services in conjunction with collaterals and team members.
  • Track billing units as they occur along with supporting documentation.
  • Drive Agency vehicle as needed.
Requirements
  • BA Degree in Health Science/Human Services preferred or equivalent years of experience.
  • High School diploma/GED equivalent required.
  • One to two years' experience working with MI populations preferred.
  • Possess and maintain a valid NYS driver's license within Agency criteria and own a vehicle.
