Job Title: Family Services Case Manager

We are seeking a highly skilled and compassionate Family Services Case Manager to join our team at Lutheran Services Florida. As a Family Services Case Manager, you will play a critical role in providing full wrap-around case management services to our clients and families, while establishing a relationship with them and living the agency's core values.

About the Role:

The Family Services Case Manager will be responsible for:

  • Providing case management services to clients and families, including assessment, planning, and implementation of services
  • Developing and maintaining relationships with clients, families, and community resources
  • Coordinating and facilitating services, including therapy, other mental health services, health and dentistry, developmental services, educational support, permanency, and safety
  • Ensuring that all case activities are accurately documented in the Florida Safe Families Network database within 48 hours
  • Maintaining physical case files in chronological order, by subject, in accordance with the standardized case file format
  • Submitting service requests to the lead agency Utilization Management Department and providing clients with timely referrals to services
  • Developing and maintaining knowledge of community resources, program eligibility requirements, key contact persons, emergency procedures, and waiting lists of available resources
  • Conducting initial and/or ongoing child safety assessments as required, and preparing initial and ongoing safety plans as necessary
  • Arranging for emergency placement, emergency medical treatment, and emergency services for children at risk
  • Conducting diligent searches for parents and family members when deemed necessary and thoroughly documenting that the effort has been made to find the parents and family members
  • Conducting home studies as required for prospective placements, and providing relevant medical, psychological, behavioral, and educational background information about the child or children to prospective caregivers as needed
  • Transporting children as needed
  • Ensuring that all Independent Living functions are completed as required
  • Attending all appointments, staff meetings, trainings, seminars, workshops, etc., as necessary and as required by the supervisor
  • Functioning as agency on-call Case Manager as scheduled
Requirements:

To be successful in this role, you will need:

  • A minimum of one year of relevant experience and certified or become certified within one year
  • A Bachelor's degree in a Human Services field, with a degree in Social Work preferred
  • Excellent written and verbal communication skills
  • Familiarity with and ability to use Microsoft Office programs Word and Excel
  • Ability to drive both locally and throughout the state in connection with the duties of this position
  • Ability to adapt to irregular hours, perform some light lifting, and be flexible to work on call 24/7 as needed
  • Certified First Aid/CPR, and crisis intervention
